Order Form
Please fill out the order form completely. If you do not have a customer number already from us, we will assign one to you with your first order after approval. Check your order and be sure to include all of the items you wish to purchase. We will process your order as received, and any additions will be handled as a separate order.
IMPORTANT: if you are paying by check or money order, you MUST PRINT out the Order Form, Complete it and Mail it in with your payment.
Sales Tax
California customers must include 8.75% sales tax on all merchandise. Tax exempt customers must submit an exemption certificate at the time the order is placed.

Returns and Exchanges
Please note that most shoes are custom made to order and are non-refundable. This is why we are able to offer you choices in styles, materials, heel heights, etc. When you place your order, please ask if it is refundable. If it is, and you need to return them, you must call for return authorization number and return address.
Returns and exchanges must be made within 14 days of shipping date, in original box, with a briefly stated reason for return, and a valid return authorization number on the box. SHOES THAT HAVE BEEN WORN OR SOILED CANNOT BE ACCEPTED FOR CREDIT OR EXCHANGE. If shoes are returned after the 14 day time period, before 30 days, and are in new condition, they will be subjected to a $15 restocking fee. No returns will be accepted after 30 days.
Glide Dance Shoes reserves the right to refuse a return. * * Purchaser is responsible for all shipping costs on refunds or exchanges. * *
There will be a $20.00 service charge on your account for any returned checks for insufficient funds. The account balance will need to be paid by Money Order or Credit Card. Your account will be charged the full invoice amount upon receipt of order.
Delivery Time
Stock shoes can be sent out the next business day after order. Special orders may take 4 - 6 weeks. Overnight and express is available at additional costs.
How fast will I get my order?
If you need your shoes by a specific date, please call 818-408-0522, and we will do our best to accommodate your request. We know you want your shoes as soon as possible and we will make every effort possible to get them to you sooner than you expect.
